China’s county medical networks need more than a video link to a city hospital

Telemedicine can place a specialist on a village clinic screen, but it cannot examine a patient, stock a medicine or keep a local clinician from leaving. China’s county medical networks aim to connect village clinics, township centres and county hospitals so patients receive appropriate care closer to home.

The organisational idea is sound: share diagnostics, staff, procurement and referral pathways across a population. Its success depends on whether capacity and authority move downward, rather than every difficult case and every skilled worker moving upward.

Define the role of each level

Village clinics need clear scopes, escalation rules and feedback after referral. Patients lose trust when they are sent onward without explanation or return with no usable plan.

Telemedicine requires a local pair of hands

Remote advice works when a trained clinician can examine, test, document and follow up. Bandwidth alone is not clinical capacity.

Align payment incentives

If revenue rewards hospital treatment while prevention is underfunded, the network will continue pulling patients inward. Budgets should support population health and continuity.

Share records carefully

Interoperable records reduce repetition, but access controls, patient consent and correction procedures are essential, especially in small communities.

Keep medicines and diagnostics reliable

A referral avoided is useful only when the local site has essential drugs, tests, maintenance and transport for specimens. Supply reliability should be publicly monitored.

Build careers outside cities

Training rotations, housing, supervision and promotion pathways can make county practice professionally viable. Compulsory short service does not create retention by itself.

Measure patient journeys

Track time to diagnosis, repeat travel, out-of-pocket cost, emergency transfers and follow-up, not only consultation volumes.

The network test

A real network makes the first point of care more capable and the whole pathway more coherent. A branded alliance with unchanged incentives merely redraws the chart.
